振动疗法治疗骨关节炎的临床研究

为探索振动疗法对骨关节疼痛的治疗效果.以42例47～78岁的中老年膝关节炎患者为观察对象,按随机原则分为治疗组33例和对照组9例.治疗组进行为期3周,每周5天,每天10分钟的全身振动治疗.对照组保持日常生活模式,不予振动治疗.所有受试者在疗程开始前和结束后均拍摄膝关节正侧位X线片,测量下肢肌力,扫描腰椎和股骨颈骨密度,填写Lysholm膝关节功能评分表,进行膝关节疼痛程度视觉类比量表评分,并进行体格检查.结果治疗组的下肢肌力上升率为78.8%、保持不变的为1.5%、下降的为19.7%;对照组下肢肌力上升率为11.1%、保持不变的为11.1%、下降的为77.78%.治疗组膝关节功能评分平均值由治疗前的87.18显著提高到91.92(P＜0.001);对照组由治疗前的75.67降低到69.44(P＜0.001).治疗组膝关节平均疼痛评分由治疗前的3.42显著降低到2.00(P＜0.001),对照组由治疗前的2.78升高到3.67(P＜0.002).上述3项观察指标治疗有效率均超过80%.表明全身振动疗法可以有效提高下肢肌力和膝关节功能,同时有效地缓解膝关节疼痛.

CLINICAL EXPERIMENTAL STUDY ON THE TREATMENT OF OSTEOARTHRITIS BY VIBROTHERAPY

In order to study the therapeutic effect of vibrotherapy on osteoarticular pains, 42 middle-aged or aged (47-to-78-year-old) cases of knee osteoarthritis were used as the experimental subjects and randomly divided into the treated group (33 cases) and the control group (9 cases); the treated group were treated by whole-body vibrotherapy for three weeks (10 min/day and 5 days/week) but the control kept the daily life mode instead of vibrotherapy. Before and after the treatment, the knee anteroposterior and lateral X-rays of all the subjects were taken; the lower-limb dynamotries were made; the lumbovertebral and femoral-head BMD were scanned; the functions and pains of the knees were scored. The results showed that the lower-limb-myodynamic ascending, no-change and descent rates of the treated and the control were: 78.8 %, 1.5 % and 19.7 % versus 11.1 %, 11.1 % and 77.78 %; the mean knee function scores of the treated were increased from 87.18 before the treatment to 91.92 after the treatment (P < 0.001), while those of the control were reduced from 75.67 before the treatment to 69.44 (P < 0.001); the mean painful scores of the treated were decreased from 3.42 before the treatment to 2.00 (P < 0.001), while those of the control were increased from 2.78 before the treatment to 3.67 (P < 0.002); all of the above-three-observed-index therapeutic effective rates of the vibrotherapy exceeded 80 %; suggesting that the whole-body vibrotherapy functions both to improve the lower-limb myodynamia and knee function effectively and to remit the knee pains effectively.
