脊髓内海绵状血管瘤的诊断与显微外科治疗

目的 探讨脊髓海绵状血管瘤（CA）的临床特点与显微外科治疗效果。方法 18例脊髓内CA患者均行MRI检查及显微外科手术治疗。对他们的临床资料进行回顾性分析。结果18例病灶全切17例，次全切1例。术后脊髓功能改善15例，无变化2例，加重1例，无手术死亡。结论 增强MRI可准确地显示脊髓内CA的大小位置，显微外科手术是其有效的治疗方法。

Diagnosis and Microsurgical Treatment of Spinal Intramedullary Cavernous Hemangiomas

Objective To explore the clinical characters of spinal intramedullary cavernous hemangiomas and the curative effect of the microsurgery on them. Methods The clinical and imaging data of 18 patients with spinal intramedullary cavernous hemangiomas undergoing microsurgery in our department from 1997 to 2005 were analyzed retrospectively. All the patients received the MRI examination. Results The definite diagnosis was made by MRI examination in all the patients. Of 18 patients with spinal intramedullary cavernous hemangiomas, 17 received total resection of the hemangiomas and 1 subtotal. The postoperative neurological function of the spinal cord was improved in 15 patients, unchanged in 2, and deterioratted in 1. No patients died from the operation. Conclusions MRI is a reliable method to diagnose spinal intramedually cavernous hemangiomas. The curative effect of microsurgery on spinal intramedullary cavernous memagiomas is good.
