腹腔镜与小切口胆囊切除术临床疗效比较

目的比较腹腔镜胆囊切除术(LC)与小切口胆囊切除术(MC)治疗胆囊疾病的临床疗效。方法随机抽取2009年7月~2011年12月LC病例50例,MC病例40例。分别对其切口长度、术中出血量、手术时间、术后住院时间、下床活动时间、术后禁食时间、肛门排气时间、抗菌药使用天数,止痛剂应用及住院费用等进行回顾性比较分析。结果同MC组相比,LC组具有切口小,术中出血量少,术后住院时间短,下床时间早,禁食时间、肛门排气时间及抗菌药使用天数短,止痛剂应用少等优势,差异有统计学意义(P<0.001);手术时间LC组明显长于MC组(P<0.05),与大多文献不符,可能与本院新开展腹腔镜手术有关;住院总费用LC组要明显高于MC组(P<0.0001)。结论 LC组临床疗效明显优于MC组,在有条件的医院值得推广。

Comparison of effect of laparoscope versus small-incision cholecystectomy

Objective To evaluate the clinical effect between laparoscope and small-incision cholecystectomy. Methods The incision size,bleeding,operation duration,postoperative hospital stay,out of bed activity time,fasting time,and exhaust time,days of antibiotics application,painkillers application and the hospitalization expenses between patients receiving laparoscopic cholecystectomy(n=50)and those who receiving small-incision cholecystectomy(n=40)from Jul 2009 to Dec 2011 were compared retrospectively. Results Compared to MC,LC had the advantages of smaller incision,less bleeding,shorter hospital stay,shorter out of bed activity time,fasting time and anal exhaust time as well as days of antibiotics application,less painkillers application and so on(P<0.0001). Operation time was longer in LC group than MC group,because of the newly development of the laparoscopy technique(P<0.05). The hospitalization expenses was significantly higher in LC group(P<0.0001). Conclusion LC is better than MC in the treatment of gall bladder diseases and is worth to be developed if possible.
