单孔腹腔镜胆囊切除术治疗慢性胆囊炎伴胆囊颈管部结石的疗效分析

目的对比分析单孔腹腔镜胆囊切除术与"两孔法"腹腔镜胆囊切除术治疗慢性胆囊炎伴胆囊颈管部结石的临床效果。方法回顾性分析2012年8月至2018年8月收治的460例慢性胆囊炎伴胆囊颈管部结石的临床资料,根据手术方法不同,分为两组,其中210例行单孔腹腔镜胆囊切除术(A组),250例行"两孔法"腹腔镜胆囊切除术(B组),对比分析两组手术时间、术中出血量、术后引流管引流量、术后住院时间和术后并发症等情况。结果手术均获成功,A组手术时间显著大于B组,差异有统计学意义(P<0.05);A组术后住院时间显著小于B组,差异有统计学意义(P<0.05);两组术中出血量、术后引流管引流量和术后并发症比较,差异均无统计学意义(均P>0.05)。结论单孔腹腔镜胆囊切除术治疗慢性胆囊炎伴胆囊颈管部结石是安全可行的方法,术后康复快,美容效果好。

Effect of laparoscopic cholecystectomy in the treatment of chronic cholecystitis with gallbladder neck duct stones

Objective To compare and analyze the clinical effects of single-port laparoscopic cholecystectomy and two-port laparoscopic cholecystectomy in the treatment of chronic cholecystitis with gallbladder neck duct stones. Methods The clinical data of 460 cases of chronic cholecystitis with gallbladder neck duct stones from August 2012 to August 2018 were retrospectively analyzed. They were divided into two groups according to different surgical methods. 210 cases underwent single-port laparoscopic cholecystectomy (group A) and 250 cases underwent “two-port” laparoscopic cholecystectomy (group B), The operation time, intraoperative bleeding volume, drainage volume, length of stay and complications of the both groups were compared and analyzed. Results The operation was successful, the operation time in group A was significantly longer than that in group B (P<0.05), the length of stay in group A was significantly shorter than that in group B (P<0.05), there was no significant difference in intraoperative bleeding volume, drainage tube flow and complications between the two groups (P>0.05). Conclusions Single-port laparoscopic cholecystectomy is a safe and feasible method for treating chronic cholecystitis with gallbladder neck duct stones.It has exhibited the characteristics of fast recovery and good cosmetic effect.
