| 摘 要： | 目的探讨初学手法小切口白内障手术（MSICS）的基层眼科医生在手术过程中发生沉核的危险因素。方法通过对参加Starr＂手把手＂MSICS培训项目的 50名基层眼科医生进行问卷调查。回顾性分析学员在学习MSICS早期手术中发生沉核的病例临床特征,手术操作过程。结果发生沉核的病例数有9例,男性6例,女性3例,年龄为62～92岁（平均77.78岁）。诊断均为老年性白内障,其中2例为过熟期,6例为大/黑核,3例合并有高度近视,1例伴有晶状体半脱位。9例MSICS手术的前囊切除方式均为截囊。结论高龄患者、过熟期、大/黑核、高度近视、晶状体半脱位等是MSICS初学者发生沉核的主要危险因素,另外开罐式截囊也是发生沉核的重要因素。 |
