踝臂指数评价高危患者外周动脉疾病的价值

目的观察高危患者合并外周动脉疾病（PAD）的发病情况,探讨踝臂指数在临床上的应用价值。方法选取393例高危患者,收集相关临床资料并检测相关生化指标,采用科林动脉硬化检测仪测量受试者的ABI值,应用SPSS13.0统计软件进行数据分析和处理。结果高危患者中合并PAD者为121例,总的患病率为30.8%,女性PAD的患病率明显高于男性（39.8%∶18.6%,P〈0.01）。PAD组年龄、总胆固醇、2型糖尿病显著高于非PAD组（P〈0.05或P〈0.01）,多因素Logistic回归分析显示,性别、年龄、2型糖尿病是高危患者伴PAD的危险因素。结论踝臂指数是筛查高危患者伴PAD的简单易行又可靠的指标,性别、年龄、2型糖尿病是高危患者踝臂指数降低的危险因素。

Evalution of ankle brachial index on high-risk patients with peripheral artery disease

Objective To observe the incidence of the peripheral artery disease（PAD）in high-risk patients and to explore the clinical value of ankle brachial index（ABI）.Methods 393 cases with high-risk patients were recruited and collected clinical data to measure biochemical indicators.Oscillometric measurement was measured the value of ABI.The date were analyzed by SPSS 13.0 software.Results There were 121 cases with PAD,the prevalence of PAD was 30.8%.Prevalence of PAD in male and female were 39.8% and 18.6%（P=0.0000）.Compared with Non-PAD group,age（65.595±10.286 years vs 60.184±9.392 years,P=0.0000）,total cholesterol（5.364±1.175 mmol/L vs 5.042±1.041 mmol/L,P=0.0068）,type 2 diabetes（55.4% vs 43.8%,P=0.0332）were significantly higher in PAD group.Logistic regression analysis showed that gender,age and type 2 diabetes were major risk factors for PAD in high-risk patients.Conclusion ABI is a feasible and reliable index for screening PAD in elderly population.Gender,age and type 2 diabetes are major risk factors of low ABI in high-risk patients.
