子宫内膜异位症患者腹腔液中自然杀伤细胞活性及细胞因子测定

目的　了解子宫内膜异位症 (内异症 )患者腹腔液中自然杀伤细胞 (NK细胞 )活性以及白细胞介素 -6(IL-6)、白细胞介素 -8(IL -8)、肿瘤坏死因子α(TNF -α)水平的变化及其意义。方法　从 3 2例内异症患者 (轻度组 13例 ,为Ⅰ ,Ⅱ期 ,重度组 19例 ,为Ⅲ ,Ⅳ期 )和 19例对照者 (对照组 ,为行输卵管结扎术和吻合术患者 )的腹腔液中获取自然杀伤细胞 ,同时收集上清液。用放射性同位素法测定NK细胞活性 ,用酶联免疫吸附法测定IL -6,IL -8和TNF -α的水平。结果　内异症轻度组和重度组NK细胞活性均显著低于对照组 (P <0 .0 1) ,重度组显著低于轻度组 (P <0 0 1) ,轻度组和重度组IL -6,IL -8及TNF -α水平均明显高于对照组 ,差异有极显著性 (P <0 0 1) ;重度组显著高于轻度组 (P <0 0 1)。结论　内异症患者腹腔液中免疫功能紊乱 ,NK细胞活性受损 ,细胞因子IL -6,IL -8,TNF -α产生过量 ,且NK细胞活性受损程度及产生细胞因子的量随疾病分期的增加而增高 ,可能共同参与内异症的发病过程

Natural killer cell activity and cytokines levels in peritoneal fluid of patients with endometriosis

Objective 　To study the change and significance of natural killer cell (NK cell)activity, interleukin-6(IL-6),interleukin-8(IL-8) and tumor necrosis factor-α(TNF-α) levels in peritoneal fluid of patients with endometriosis. Methods 32 patients with endometriosis were divided into two groups(mild group: 13,stageⅠ or Ⅱ; severe group: 19,stage Ⅲ or Ⅳ ). 21 controls were also included. The NK cell activity and levels of IL-6,IL-8 and TNF-α in peritoneal fluid were measured by radioisotope method or enzyme linked immunosorbent assays. Results The NK cell activity in peritoneal fluid from two endometriosis groups was significantly lower than that in controls (P＜0.01), and NK cell activity of severe group was significantly lower than that of mild group.The concentrations of IL-6,IL-8 and TNF-α were significantly higher in two endometriosis groups than those in controls (P＜0.01), with significant differences between two endometriosis groups (P＜0.01).Conclusions The immune function of peritoneal fluid of patients with endometriosis is impaired. The NK cell activity is lower,the levels of IL-6,IL-8 and TNF-α are higher. The clinical stage of endometriosis is positively correlated to cytokine level and negatively correlated to NK cell activily. They may participate in the pathogenesis of endometriosis.
