获卵少周期卵体外受精方式的临床分析

目的:探讨获卵少周期卵体外受精方式的选择及临床结局。方法:回顾性分析行IVF-ET治疗的807例获卵数≤5的患者的资料,比较常规IVF组和ICSI组的临床结局。结果:ICSI组正常受精率、可用胚胎率均显著高于常规IVF组(P<0.05),多PN率显著低于常规IVF组(P<0.05),而优质胚胎率、着床率、妊娠率、累积妊娠率、活产率、周期取消率、完全受精失败率两组间差异无统计学意义,但着床率、妊娠率、累积妊娠率、活产率常规IVF组均高于ICSI组。结论:当获卵数少时ICSI可提高正常受精率和可用胚胎率,降低多PN率,但并不改善临床妊娠结局。因此,ICSI并不能优化患者周期,获卵少周期卵并不推荐ICSI受精方式。

Clinical analysis on the method of in vitro fertilization in patients with few oocytes retrieved

Objective:To explore the choice of in vitro fertilization(IVF) method and clinical outcome of patients with few oocytes retrieved.Methods:The data of 807 patients with few oocytes retrieved(≤5) who were treated with IVF-ET were analyzed retrospectively.The clinical outcomes of conventional IVF group and intracytoplasmic sperm injection(ICSI) group were compared.Results:The normal fertilization rate and utilization rate of embryos in ICSI group were significantly higher than those in conventional IVF group(P<0.05),while the rate of multiple PN was significantly lower than that in conventional IVF group(P<0.05).There was no significant difference in the rate of high quality embryos,the implantation rate,the pregnancy rate,the cumulative pregnancy rate,the live-birth rate,the cycle cancellation rate and the complete fertilization failure rate between the two groups,but the implantation rate,the pregnancy rate,the cumulative pregnancy rate and the live-birth rate in IVF group were higher than those in ICSI group.Conclusion:When the number of retrieved oocytes is few,ICSI can increase the normal fertilization rate and utilization rate of embryos,reduce the rate of multiple PN,but it can't improve the clinical pregnancy outcome.Thus,ICSI can't optimize the cycles of patients,ICSI is not recommended for the patients with few oocytes retrieved.
