喉罩与气管插管用于全麻腹腔镜下腹部及盆腔手术的比较

摘    要:
目的:比较喉罩(LMA)和气管插管(ET)用于全麻腹腔镜下腹部及盆腔手术期间的通气可靠性和血流动力学的影响。方法:100例在全麻下行腹部及盆腔手术患者随机分为喉罩组和气管插管组。记录麻醉诱导前(T)、诱导后(T)、置入即刻(T)、置入后3min(T)、手术中(T)、拔除即刻(T)、拔除后3min(T)的SBP、DBP、HR、SpO变化。并记录拔除前后呛咳、躁动、反流误吸等情况。结果:两组SBP、DBP、HR在诱导插管即刻、插入后3min、拔除即刻、拔除后3min比较差异有统计学意义,P〈0.01;ET组SBP、DBP、HR在置入即刻、拔除即刻、拔除后3min较基础值比较差异有统计学意义,P〈0.01。结论:全麻喉罩通气用于腹腔镜下腹部及盆腔手术优于气管插管,易于维持血流动力学稳定。

Abstract:
Objective:Use in the peritoneoscope underbelly and on pelvis general anesthesia period ventilation reliability and the blood stream dynamics influence compared with LMA and ET.Method:100 examples were divided into the LMA group and the ET group in the general anesthesia downward abdomen and on pelvis patient.After before record anaesthesia induction,(T),induction (T),posts instantly (T),posts 3min (T),in the surgery (T),wiping out instantly (T),wiping out 3min (T) SBP,DBP,HR,SpO change.And records around wiping out to choke coughs,moves restlessly,the regurgitation to attract and so on situations by mistake.Result:Two group of SBP,DBP,HR after induction intubation instantly,insertion 3min,wiping out instantly,wiping out the 3min comparison difference has statistics significance,P0.01;ET group SBP,DBP,HR after posting instantly,wiping out instantly,wiping out 3min compare the foundation value comparison difference to have statistics significance,P0.01.Conclusion:The general anesthesia LMA ventilation uses in the peritoneoscope underbelly and on pelvis surpasses ET,easy to maintain the blood stream dynamics to be stable.
