不同方法对人群肺弥散功能的检测价值研究

摘    要:
目的探讨一口气法与重复呼吸法对人群肺弥散功能的检测价值。方法选取2010年1月至2013年10月我院收治的34例间质性肺疾病(ILD)患者以及36例COPD患者,分别将其设为ILD组以及COPD组,另外选取33名健康人设为对照组。分别采用一口气法与重复呼吸法对其肺弥散功能进行检测。结果ILD组和COPD组的肺功能指标分别与对照组相比,差异均有统计学意义(P〈0.05)。对照组采用重复呼吸法测定D-CO%pred的平均值[(78.91±2.a4)%]低于80%。校正前,3组用一口气法及重复呼吸法测得的DLCO%pred、DLCO/AV%pr出相比,差异均有统计学意义(P〈0.05)。COPD组校正后采用2种测试方法测得的DLCO%pred、DLCO/AV%pred与ILD组相比,差异均有统计学意义(P〈0.05)。一口气法的检测时间平均为(11.28±0.14)S,明显短于重复呼吸法的检测时间[(86.13±3.25)S,P〈0.05]。重复呼吸法测得的DLCO/AV%pred与测试时间的相关性(r=-0.661,P〈0.01),相比于DLC0%pred与测试时间的相关性(r=0.391,P〈0.01)明显增高,差异有统计学意义(P〈0.05)。校正前采取2种不同方法测得ILD组轻、重度及COPD组轻度的DLCO%pred相比,差异均有统计学意义(P〈0.05)。校正后ILD组轻、重度患者采取2种不同方法测得的DLCO%pred相比,差异均有统计学意义(P〈0.05)。结论相比于一口气法,对老年患者、不能屏气者、存在ILD以及COPD患者而言,采用重复呼吸法检测肺弥散功能无需受试者屏气,测量结果可能具有更高的参考价值。

Abstract:
Objective To investigate value of different methods for the detection of pulmonary diffusing capacity of the population. Methods 34 cases of interstitial lung disease (ILD) and 36 cases of chronic obstructive pulmonary disease (COPD) from January 2010 to October 2013 in our hospital were seceted respectively,and were set to ILD group and COPD group, while 33 cases of healthy people were seceted as control group. Breathing method and repeat breathing method were used to detect pulmonary diffusion function of patients. Results The lung function in ILD group and COPD group was statistically different from control group ( P d0.05). DI CO%pred in control group by repeat breathing method was (78.91 ±2.24) %, less than 80%. Before correction, DLCO; pred, DLCO/AV% pred by two different methods were statistically different in three groups ( P〈 0.05 ). After correction, DLCOG pred, DLCO/AV;pred by two different methods between COPD group and ILD group (P 〈 0.05). The average time by breathing method was (11.28 ± 0.14) s, significantly shorter than that by repeat breathing method r(86.13; 3.25) s, P d0.05]. The correlation between DLCO/AV; pred by repeat breathing method and test time ( r = -0. 661, P ; 0.01) was significantly increased compared to the correlation between DLCO ;/00 pred and test time ( r =0. 391, P d0.01). Before correetion,DLCO % pred by two different methods was statistically different in mild and severe ILD group, mild COPD group ( P〈 0.05). After correcting, DLCO% pred by two different methods was statistically different in mild and severe ILD group ( P 〈 0.05). Conclusions Compared to breath method, elderly patients who can not breath,and together with interstitial lung diseaseand chronic obstructive pulmonary disease, detection of pulmonary diffusion by repeat breathing method is without subjects, the measurement results may have more high reference value.
