139例食管癌X线造影与CT对比分析

目的 :通过食管癌 X线片与 CT的对比研究 ,探讨诊断食管癌外侵的方法。方法 :选择资料完整的 395例患者 ,作 X线食管钡剂造影和食管 CT扫描 ,以相应外侵判断指标判读 ,测量病变长度、外侵深度、外侵范围、管壁厚度及淋巴结转移情况 ,分别对比分析。结果 :二者相比 ,X线片病变长度超过 5 cm者外侵率 2 8.6 % ,超过 7cm者外侵率 31.3%。 CT片病变长度超过 5 cm者外侵率 5 3.6 % ,超过 7cm者 ,外浸率 5 3.1%。二者对比差异有显著性 (P<0 .0 1)。 CT对食管肿瘤外侵的检出率为 48.9% ,对纵隔内淋巴结转移检出率为 38.1%。而食管钡剂 X线摄片发现肿瘤外侵为 2 5 .9% ,对纵隔内淋巴结转移检出率为零。二者对比差异有显著性 (P<0 .0 1)。结论 :食管癌手术前、放射治疗前常规作 CT检查可明确病变长度、宽度、外侵的程度、浸润方式、外侵部位及纵隔内淋巴结转移情况 ,对治疗方法的选择及指导放射治疗视野有重大参考价值

A comparative study between X-ray and CT diagnose esophageal carcinoma

Objective: It is to comparative study esophageal carcinoma between X ray and CT so as to estimate the invasive depth. Methods: Computer tomography,gas and barium double contrast radiography were taken in 139 cases with esophageal carcinoma. The length, invasive depth,thicker, and width and locality lymphnodes of tumor segment were measured. Results: In x ray group, the infiltration rates of length of tumor segment over 5cm and 7cm were 28.6% and 31.3%. In CT group, the infiltration rates of the length of tumor segment er 5cm and 7cm were 53.6% and 53.1%, the carcinoma infiltrated through adventitia to the adjacent structure and locality lymphnodes metastasis was markedly increased. Conclusion: The computer tomography sversal invasion in esophageal carcinoma is related to the invasive depth of tumor segment. From these results the comprehensive evaluating criteria about esophageal carcinoma was determined, which is of important clinical value for diagnosis, selection of surgery or radiotherapy.
