倾斜试验加硝酸甘油舌下含化诊断血管迷走性晕厥的临床研究

目的 探讨倾斜试验加硝酸甘油(NTTT)舌下含化诊断血管迷走性晕厥的安全性、敏感性和特异性.方法 28例不明原因晕厥患者进行倾斜试验并以26例正常人作对照组.结果 实验组基础倾斜试验(BTTT)阳性6例(21.43%),对照组基础倾斜试验1例阳性(3.85%),舌下含服0.5 mg硝酸甘油后实验组有9例阳性(60%),对照组阳性2例,两组差异有统计学意义(P<0.05),实验组两种试验阳性率差异亦有统计学意义(P<0.01).NTTT敏感性为60%,特异性为92.3%.结论 倾斜试验采用70°角,持续45 min,含服0.5 mg硝酸甘油诊断血管迷走性晕厥安全性、敏感性和特异性较高.

Clinical study of the sublingual nitroglycerin administration head-up table tilt test in diagnosis of vasovagal syncope

Objective To investigate the security,sensibility and specificity of sublingual nitroglycerin administration during head-up table tilt as a simple pratical test for the vasovagal syncope.Methods Twenty eight cases of syncope with unknown causes and 26 healthy controls were analysed.Results Six cases in experimental group showed positive reaction to BTTT(21.43%),while one in control group was positive(3.85%).After 0.5 mg of sublingual nitroglycerin,9 positive occurred in experiment group(60%),compared to control group,there was statistically significant difference(P<0.05).The difference between BTTT and NTTT was also significant in experiment group(P<0.01).The sensibility and specificity of NTTT were 60% and 92.3%.Conclusion Head-up table tilt test at 70 degrees for 45 minutes and sublingual admiestration of 0.5 mg nitroglycerin should have good value of security,sensibility and specificity in diagnosis of vasovagal syncope during the clinical application.
