应激性高血糖对危重症预后的影响

目的探讨应激性高血糖对危重症患者的预后影响。方法通过清远市人民医院历年接诊的346例急诊危重症病例作为分析对象,其中伴应激性高血糖病294例为治疗组,无伴应激性高血糖病52例为对照组;同时,对治疗组合并应激性高血糖的急诊危重症患者,根据每日晨空腹血糖平均值分为:A组血糖≤10.0mmol/L;B组血糖≥10.0mmol/L,所有资料进行回顾性分析。结果治疗组并发症率及病死率均比对照组高,差异有统计学意义;且治疗组的A组危重症住院时间、机械通气时间、多器官功能衰竭综合征(MODS)发生率以及病死率比C组的理想,差异有统计学意义。结论应激性高血糖对危重症患者的预后不利,且高血糖程度越高,对预后越不利。

Impact of Stress Hyperglycemia on Critical Disease Prognosis

Objective To research into the impact of stress hyperglycemia on critical disease prognosis.Methods Analyze the 346 emergency critical cases treated in Qingyuan people＇s hospital in the past years,of which 294 cases of stress hyperglycemia are grouped into a treatment group,52 cases without stress hyperglycemia are grouped into a comparison group.In the meantime,for emergency critical patients with stress hyperglycemia in the treatment group,further divide them into 2 groups according to their daily average fasting blood glucoses：Group A glucoses≤10.0mmol/L.Group B glucoses≥ 10.0mmol/L.Carried out retrospective analysis on all data.Results the complications ratio and death rate of the treatment group was higher than those of the comparison group.The difference has statistical significance.The critical cases in-patient time,mechanical ventilation time,occurrence rate of Multiple Organ Dysfunction Syndrome（MODS）and death rate of Group A of the treatment group was better than those of Group C.The differences have statistical significance.Conclusion The prognosis of stress hyperglycemia cases is poor,and the higher the hyperglycemia level is,the worse the prognosis will be.
