摘    要:新生儿脑缺血缺氧是围产期导致新生儿致死致残的重要原因 ,重度患儿多遗留运动或感觉障碍。一种新型的血管活性物质肾上腺髓质素 (adrenomedullin ,AM )除具有很强的扩血管作用外 ,最近的研究还发现其在脑缺血缺氧的病理生理学中起重要作用 :不仅有特殊的保护特性 ,还可能是新生儿窒息早期出现脑缺血缺氧等并发症的一种诊断标志物 ,而患儿此时的临床表现和头颅B超检查等仍为阴性。外源性给予或内源性刺激AM的产生可能为临床上新生儿脑缺血缺氧的早期诊断、治疗和预防提供一种新的途径。

Abstract:Hypoxi c-ischemic brain injury in newborn infants is a significant cause of neonatal d eath and disability during perinatal stage. Children with severe hypoxic-ischem ic brain injury often leave motor or sensomotor dysfunctions. Adrenomedullin (AM ) is a novel vasoactive substance. Apart from its great effect of vessel dilatio n, the recent studies have found that it plays an important role in the pathophy siology of hypoxic-ischemic brain injury. It not only has the special protectiv e effect, but may also be a diagnostic indicator in early hypoxia and ischemia, and at this time, the clinical manifestation and cerebral ultrasound in children are still negative. The development of AM from ectogenic or endogenetic stimula tion may provide a novel approach in the diagnoses, treatment and synteresis of neonatal cerebral ischemia in clinical practice.
