预防人工髋关节置换术并发症的护理

目的探讨预防人工髋关节置换术并发症的护理措施。方法回顾性分析1995—01／2007—01采用骨水泥型与非骨水泥型人工髋关节置换术，治疗老年股骨颈骨折46例和股骨头缺血坏死24例的资料。结果1例全髋关节置换术后脱位，经及时复位后未再脱位；1例髋关节早期感染，经关节冲洗引流加强抗感染后，伤口愈合；1例术后肺部感染呼吸衰竭死亡。术后随访时间1～12年，平均随访2．5年。髋关节功能采用Harris评分：优（〉90分）34例、良（80～89分）23例、尚可（70～79分）11例，差（〈70分）1例，优良率82．6％。结论预防人工髋关节置换术并发症的护理是一个有效、简单和安全的措施，能减少病死率和并发症发生率，减少住院时间和费用。

Nursing on Prevention of Complication of Hip Joint Replacement

Objective To explore nursing intervention of the prevention of the complication of the hip joint replacement.Methods From January 1995 to January 2007,70 patients (46 patients suffered from femoral neck fracture and 24 patients suffered from avascular necrosis of the femoral head).Carried out hip joint replacement with bone cement and cementless were retrospectively analyzed. Results All patients were followed up clinically and radiographically from 1 to 12 years with an average of 2.5 years.According to Harris scoring,the hip functions were excellent(>90)in 34 cases,good(80~89)in 23 cases,fair(70~79)in 11 cases. poor(<70)in 1 case. The total excellent and good rate was 82.6%.Conclusion Nursing is an effective,simple and safe way to prevent the complication of the hip joint replacement. It is a key point to decrease the mortality and rate of complication. And the average hospitalization cost of patients decreases as well.
