羟基喜树碱联合顺铂治疗晚期食管鳞癌临床观察

目的观察羟基喜树碱(HCPT)联合顺铂(PDD)组成的HP方案对晚期食管鳞癌的近期疗效和不良反应。方法43例晚期食管癌患者采用HP方案:羟基喜树碱(HCPT)6 mg/m2,d1~5,静脉点滴;顺铂(PDD)20 mg/m2,d1~5,静脉点滴。21天为1个周期。化疗2个周期后评价近期疗效和不良反应。结果43例均可评价,CR 5例(11.6%)、PR 17例(39.5%)、SD 15例(34.9%)和PD 6例(14.0%)。近期总有效率51.2%。主要不良反应为骨髓抑制和胃肠道反应,多为Ⅰ~Ⅱ度,患者均可耐受,不影响治疗正常进行。结论HP方案治疗晚期食管癌近期疗效好,不良反应少,值得临床推广应用。

Objective of Curative Effect of Combination Chemotherapy with HCPT and Cisplatin for Advanced Esophageal Cancer

Objective To evaluate the efficacy and safety of the combination of 10-hydroxycamptothecin(HCPT) and cisplatin(PDD) in the treatment of advanced esophageal cancer.Methods The chemotherapy was used HCPT 6 mg/m~2 daily for five days and PDD 20mg/m~2 daily for five days,one cycle was 21 days for 43 patients with advanced esophageal cancer.Results Object response rates were obtained in all patients.The overall response rate was 51.2 % with 5 complete response(CR) and 17 partial response(PR).The adverse reaction mainifested mainly the yeaction of gastroiontestinal tract and the suppression of bone marrow.there as no treatment-related death.Conclusion HCPT plus PDD is active and tolerable in treatment of advanced esophageal cancer,it is valuable to be used extensive in clinic.
