产后抑郁与分娩前后血清同型半胱氨酸、雌二醇水平变化的相关性分析

摘    要:目的分析产后抑郁与分娩前后血清同型半胱氨酸(Hcy)、雌二醇(E2)水平变化的相关性。方法选取我院2018年7月至2019年7月收治的120例初产妇,在分娩后7天时采用爱丁堡产后抑郁量表(EPDS)评估产妇的抑郁状态,根据评估结果分为产后抑郁组(n=15)和非抑郁组(n=105)。比较两组产妇的一般资料及分娩前、分娩后7天的血清Hcy、 E2水平,分析EPDS评分与血清Hcy、 E2水平的相关性。结果 120例产妇中,有15例(12.5%)发生产后抑郁。产后抑郁组的受教育程度显著低于非抑郁组(P <0.05)。分娩前,两组的血清Hcy、 E2水平比较无统计学差异(P>0.05);分娩后7天,产后抑郁组的血清Hcy水平显著高于非抑郁组,血清E2水平显著低于非抑郁组(P均<0.05)。EPDS评分与分娩后血清Hcy水平呈正相关(r=0.233, P <0.05),与分娩后血清E2水平呈负相关(r=-0.691, P <0.05)。结论分娩后血清Hcy水平升高、 E2水平降低可能是产后抑郁的危险因素,检测分娩后Hcy、 E2水平有助于产后抑郁症的预测。

Abstract:Objective To analyze the correlation between postpartum depression and changes of serum homocysteine(Hcy) and estradiol(E2) levels before and after delivery. Methods 120 primiparas admitted to our hospital from July 2018 to July 2019 were selected.The Edinburgh Postnatal Depression Scale(EPDS) was used to evaluate the depression state of puerperas at 7 days after delivery. According to the evaluation results, they were divided into postpartum depression group(n = 15) and non-depression group(n = 105). The general data and the serum Hcy and E2 levels before delivery and 7 days after delivery were compared between the two groups. The correlation between EPDS score and serum Hcy and E2 levels was analyzed. Results Among the 120 puerperas, 15 cases(12.5%) had postpartum depression.The education level of the postpartum depression group was significantly lower than that of the non-depression group(P <0.05). Before delivery, no statistical difference was found in the serum Hcy and E2 levels between the two groups(P >0.05). 7 days after delivery, the serum Hcy level of the postpartum depression group was significantly higher than that of the non-depressed group, and the serum E2 level was significantly lower than that of the non-depressed group(both P <0.05). EPDS score was positively correlated with postpartum serum Hcy level(r = 0.233, P <0.05), and was negatively correlated with postpartum serum E2 level(r =-0.691, P <0.05). Conclusions The increase of serum Hcy level and the decrease of serum E2 level after delivery may be risk factors for postpartum depression. Detecting Hcy and E2 levels after delivery can help predict the postpartum depression.
