阴式与腹腔镜手术治疗对子宫肌瘤患者卵巢功能、性功能及预后的影响

目的探讨阴式与腹腔镜手术治疗对子宫肌瘤患者卵巢功能、性功能及预后的影响。方法选择2018年1月至2019年5月我院收治的80例子宫肌瘤患者,随机平均分为两组。试验组行阴式子宫肌瘤剔除术(VM),对照组行腹腔镜子宫肌瘤剔除术(LM)。比较两组的围术期相关指标、卵巢功能、性功能、并发症情况。结果试验组的术中出血量、住院时间、手术时间均少于对照组(P <0.05)。两组的肌瘤剔除数量、并发症发生率比较无统计学差异(P>0.05)。术后,试验组的E2水平低于对照组,LH、 FSH水平及性唤起、性心理、性行为异常评分高于对照组(P均<0.05)。结论 VM、 LM治疗子宫肌瘤的有效性与安全性相似,前者具有出血量少、手术时间短、恢复快、对卵巢功能和性功能影响小等优势。

Influence of Vaginal and Laparoscopic Surgery on Ovarian Function,Sexual Function and Prognosis of Patients with Hysteromyoma

Objective To explore the influence of vaginal and laparoscopic surgery on ovarian function, sexual function and prognosis of patients with hysteromyoma. Methods 80 patients with hysteromyoma admitted to our hospital from January 2018 to May 2019 were selected and randomly divided into two groups equally. The experimental group received vaginal myomectomy(VM), and the control group received laparoscopic myomectomy(LM). The perioperative related indicators, ovarian function, sexual function and complications were compared between the two groups. Results The intraoperative blood loss, hospitalization time and operation time in the experimental group was less than that in the control group(P <0.05). No statistical difference was found in the number of myomectomy and the incidence of complications between the two groups(P >0.05). After operation, the E2 level of the experimental group was lower than that of the control group, the LH and FSH levels and the scores of sexual arousal, sexual psychology and abnormal sexual behavior were higher than those of the control group(all P <0.05). Conclusions VM and LM in the treatment of hysteromyoma have similar efficacy and safety, but the former has the advantages of little blood loss, short operation time, quick recovery, and little influence on ovarian function and sexual function.
