同种异体原位心脏移植术患者2例术后肾功能的维护

目的：就2例心脏移植患者术后肾功能的维护作一探讨。方法：保持良好氧供；加强血管活性药物的输注管理及少尿时的护理。结果：病例1，术后肾功能恢复顺利，病例2，术后尿量与环孢素A的血液浓度呈明显负相关；经统计学处理，相关系数为-0．7。结论：术后肾功能维护的关键是维持满意尿量。良好氧供、适当血管活性药物治疗、及时处理少尿是有力措施。对于术前肾功能不全或年龄较大患者，尤其密切观测尿量及环孢素A血液浓度，根据尿量及时调整环孢素A的用法及用量。

Protection of Renal Function after Heart Transplantation-A Report of 2 Cases

To study the protection of renal function after heart transplantation in 2 cases. Methods: Keeping goodoxygen supply, enhancing the administration of vasomotor and nursing during oliguresis. Results: In case 1,the renal function was repaired smoothly. In case 2,the urine volume was negatively related to serum concentration of Cyclosporine A. The correlation coefficent was -0. 7. Conclusions: The sticking point of the protection of renal function after operation is to keep the urine volume properly. Good oxygen supply, proper vasomotor application and dosage and prompt intervention of oliguria are indispensible measures. To patients who were old or had renal disfunction before operation,it is especially important to observe the urine volume and the serum concetration of Cyclosporine A, and to adjust the application and dosage instantly according to the urine volume.
