早期胃癌内镜下诊断分析

摘    要:
目的:探讨早期胃癌内镜下的诊断意义.方法:选择我院2007年10月至2010年10月早期胃癌患者42例,以上患者均经病理组织学检查证实.本组患者均行内镜检查和病理组织学检查.采用活检钳钳取组织,直径小于1.0cm全部取材,直径大于1cm较多取材.取材组织做病理学检查.结果:①病变部位和胃镜下形态:胃窦10例,胃体9例,胃角15例,贲门8例.胃镜下形态:Ⅰ型1例;Ⅱa型1例;Ⅱb型5例;Ⅱ型24例;Ⅲ型11例.②病理组织学检查:腺癌19例,占45.2%.粘液细胞癌12例;印戒细胞癌1例.鳞癌10例.结论:早期胃癌内镜下形态多表现为浅表凹陷和溃疡型,胃壁任何部位都可能发生.重视内镜检查并准确行粘膜活检有助于尽早发现早期胃癌,有助于提高患者预后.

Abstract:
Objective:To explore the diagnostic significance of early gastric cancer under endoscopy.Methods:in our hospital from 2007 October to 2010 October in 42 cases with early gastric cancer,more patients were confirmed by histopathologic examination.This group of patients underwent endoscopy and histopathological examination.Using biopsy forceps to take a tissue,diameter less than 1.0cm all material,diameter of more than 1cm more material.Based on histopathological examination.Results:the site of lesion and gastroscopic morphology:10 cases of gastric antrum,9 cases of gastric body,gastric angle in 15 cases,with 8 cases.Gastroscopic morphology:type 1 cases;II type 1;type 5;type 24;type 11 cases.The histopathological examination:19 cases of adenocarcinoma,45.2%.Mucous cell carcinoma 12 cases;signet ring cell carcinoma in 1 cases.Squamous cell carcinoma in 10 cases.Conclusion:early gastric endoscopic morphology multiple manifestations of superficial depressed and ulcer,gastric wall may occur anywhere.Attach importance to endoscopic examination and accurate line mucosal biopsies are useful in early detection of early gastric cancer,helps to improve the patients prognosis.
