度洛西汀与文拉法辛联合米氮平治疗难治性抑郁症的对照研究

目的:比较度洛西汀与文拉法辛联合米氮平治疗难治性抑郁症的疗效及安全性。方法:63例门诊和住院患者,随机分为度洛西汀联合米氮平31例为研究组,文拉法辛联合米氮平32例患者为对照组,治疗8周。采用17项汉密尔顿抑郁量表(HAMD17)评价疗效,药物不良反应评价用副反应量表(TESS)。结果:两组患者治疗后HAMD较治疗前均有显著性降低(P<0.01);第8周,研究组患者与对照组有效率分别为67.9%和71.4%,两组患者疗效差异无统计学意义(P>0.05)。不良反应方面,对照组患者血压升高的发生率高于研究组,差异有统计学意义(P<0.05)。结论:度洛西汀联合米氮平对于难治性抑郁症有明显疗效,文拉法辛联合米氮平治疗血压升高的发生率要高于度洛西汀联合米氮平治疗。

Comparative study of Duloxetine combined with Mirtazapine,Venlafaxine combined with Mirtazapine in treatment of refractory depression

Objective：To compare efficacy and safety of Duloxetine combined with Mirtazapine, and Venlafaxine combined with Mirtazapine in treatment of refractory depression. Methods：63 outpatients and inpatients were randomly divided into study group （treated with Duloxetine combined with Mirtazapine for 8 weeks, 31 cases） and control group （treated with Venlafaxine combined with Mirtazapine for 8 weeks, 32 cases）. 17-item Hamilton depression rating scale （HAMD17） was used to evaluate the efficacy, adverse drug reactions were evaluated with treatment emergent symptom scale （TESS）. Results： After the treatment, HAMD scores of the two groups both decreased significantly （P〈0.01）. On the 8th week, the effective rates of study group and control group were 67.9% and 71.4%, respectively, and the difference had no statistical significance （P〉0.05）. The incidence rate of blood pressure elevation of control group was higher than that of study group, and the difference had a statistical significance （P〈0.05）. Conclusions： Duloxetine combined with Mirtazapine has a obvious curative effect in treating refractory depression; however, the incidence rate of blood pressure elevation of Venlafaxine combined with Mirtazapine in the treatment of refractory depression is higher than that of Duloxetine combined with Mirtazapine.
