抗HBe阳性慢性肝炎患者的HBV感染状态研究

摘    要:本文结合肝内HBsAg与HBcAg的表达情况对15例抗-HBe阳性慢性肝炎患者进行了肝细胞内HBV DNA原位杂交分析。发现其HBV感染状态可分为3种类型;①HBV活动复制型(4例,26.7%),肝内HBV DNA、HBsAg、HBcAg三者或HBsAg、HBcAg阳性;②HBV不完全复制或表达型(8例,53.3%),肝内HBV DNA、HBsAg阳性;③HBV非复制型(3例,20.0%),肝内HBV DNA及HBV标志均阴性。提示:在血清HBeAg向抗-HBe转换的病例中,HBV感染状态不尽相同,部分患者仍存在HBV活动性复制,少数病例的HBV复制已停止,半数病例处于由活动性复制相向非复制相过渡阶段。

Abstract:To investigate HBV infection status in anti-HBe positive Chinese patients with chronic hepatitis,intrahepatocelullar HBV DNA was visualised by in situ hybridization technique,coupled with demonstration of intrahepatic HBsAg and HBcAg According to presence or absence of HBV DNA,HBsAg and HBcAg,of 15 cases three subgroups were identified 1) 4 cases of HBV active replication,characterized by intrahepatic HBV DNA,HBsAg and HBcAg positive; 2) 8 cases of HBV incomplete replication or antigen expression,defined by intrahepatic HBV DNA and HBsAg positive,3)3 cases of HBV inactive replication,identified by intrahepatic HBV DNA,HBsAg and HBcAg negative.All findings strongly suggest that different HBV infection status could be distinguished among the Chinese cases of seroconversion from HBeAg positive to anti-HEe:a portion of cases were still undergoing HBV active replication,while a small number of cases stopped replication,but HBV infection in half of the cases was moving rom virus active replication to inactive replication phase.
