| 摘 要： | 目的探讨困难气管切开术在头颈肿瘤围手术期气道管理中的临床应用价值。方法 22例头颈肿瘤合并困难气道患者均采用气管切开术管理气道,同时对气管切开术后患者有无呼吸困难、有无通气功能障碍、血氧饱和度等进行观察,随访6～24个月。结果 22例困难气道患者经气管切开术后实施气道管理后,原有的通气困难明显改善,平均血氧饱和度(SaO2%)均95%,1例因气管软化发生拔管困难,总有效率95.45%,1例于随访后的第8个月,1例于随访后的第13个月,因肿瘤复发临床死亡。结论困难气管切开术是头颈肿瘤患者围手术期气道管理的一种安全、有效方法。

| Abstract: | Objective To investigate the clinical application of difficulty tracheotomy during operation of head and neckneoplasms.Meththods 22 difficulty airway patients in head and neck neoplasms were perfomed tracheotomy and observed their breathing, ventilating function and oxygen saturation, all of them were followed up for 6--24 months. Results 22 difficulty airway patients with airway management by lracheotomy, their difficulty ventilation were improved greatly, average oxygen saturation was greater than 95%, lease was difficulty in detubation with tracheomal acia, the overall effective rate was 95.45%, 2 cases recurred and died after 8 months and 13months. Conclusion Difficulty tracheotomy is a safe and effective method during operation of head and neck neoplasms. |
