引流管头端位置对钻孔引流治疗慢性硬膜下血肿疗效的影响

目的:探讨钻孔引流术治疗慢性硬膜下血肿后引流管头端放置位置对疗效的影响.方法:92例慢性硬膜下血肿患者,近4年收治的患者作为研究组(48例),4年前收治的作为对照组(44例).研究组钻孔冲洗后引流管头端朝向额方向放置,对照组钻孔冲洗后引流管头端置于血肿腔最低处,比较两组患者术后拔管时间,引流管梗阻发生率,颅内积气发生率,硬膜下残留液体量.结果:与对照组相比,研究组术后拔管时间缩短,引流管梗阻发生率低,无发生颅内积气,血肿残留量少.结论:钻孔引流术后引流管头端置于额叶方向可以减少术后并发症,缩短患者术后恢复时间,改善患者预后.

Therapeutic effect of different drainage catheter location during trepanation and drainage treatment on chronic subdural hematoma

Objective： To analyze the therapeutic effect of different drainage catheter location during trepanation and drainage treatment on chronic subdural hematoma （CSDH）. Methods： Ninety two patients with CSDH were analyzed retrospectively. Patients were divided into control group （44 cases） with drainage catheter located on the lowest in hematoma cavity and research group （48 cases） with drainage catheter located on the frontal lobe. The extubation time, rate of drainage tube obstruction and intracranial pneumatoeele, as well as the residual volume of subdural liquid of the two groups were compared. Results： Compared with the control group, catheter on the frontal lobe cortex postoperatively displayed shorter extubation time, lower rate of drainage tube obstruction, rewer residual volume of hematoma and no intracranial pneumatocele occurrence.
