颈椎管狭窄症纤维性椎管MRI测定的意义

摘    要:目的 通过对颈椎管狭窄病人MRI中矢位的测量以表明纤维性增生在椎管狭窄中的意义。方法 颈椎管狭窄病人 97例 ,其中手术病人 4 0例。在T1 加权像测量 :椎体矢状径 ,骨性椎管矢状径。在T2 加权像测量 :纤维性椎管矢状径。并将所得数据进行统计学处理 ,以比较纤维性椎管和骨性椎管在椎管狭窄症中的作用以及在临床中意义。结果 纤维性椎管的矢状径在各个节段均明显小于相应的骨性椎管矢状径 (P <0 .0 0 1)。手术组的骨性椎管中矢状径在各个节段均小于非手术组 ,但仅有两个节段对比有显著性差异 (P值分别小于 0 .0 5和 0 .0 1)。而两组的纤维性椎管中矢状径在各个节段对比均有显著性差异 (P值均小于 0 .0 0 1)。结论 造成椎管狭窄、脊髓受压的主要和直接的原因是纤维性椎管狭窄。而纤维性椎管的构成主要是硬膜囊前面的椎间盘和后面的黄韧带 ,它们前后夹击使脊髓受到明显的压迫。只有MRI能准确地反映骨性和纤维性椎管狭窄的部位、程度及髓内改变

Abstract:Objective To investigate the meaning of measurement of sagittal diameter of cervical spinal canal under MRI in stenosis of cervical spinal canal.Methods The sagittal diameter of vertebra body and osteal spinal canal were measured on T 1 weighted imaging in 97 patients with cervical stenosis, in which 40 patients underwent operation.Results The sagittal diameters of fibrous spinal canal were significant smaller than that of osteal spinal canal(P<0.001).The sagittal diameters of osteal spinal canal in operative group were smaller than that in non-operative group, but there was a significant difference only in two segments. All the sagittal diameters of fibrous spinal canal in operative group were significant smaller than that in non-operative group.Conclusion Fibrous spinal canal stenosis is the main and direct reason which cause the compression of spinal cord (MRI can show the degree of cervical spinal canal stenosis and abnormal changes accurately).The accurate degree of cervical spinal canal stenosis and abnormal changes in spinal cord can be shown in MRI.
