| 摘 要： | 目的 分析我院静脉输液配制中心不合理用药医嘱,促进临床合理用药.方法 采用回顾性分析方法,对我院静脉输液配置中心2012年1-6月审核发现的不合理用药医嘱进行统计、分析.结果 48 569例医嘱中,共审查出不合理用药医嘱423例(占0.88％),主要是溶媒选择不合理47例,用药频率不当132例,配伍禁忌95例,用法用量不合理79例,联合用药不合理38例,输入医嘱错误32例.结论 临床用药仍存在问题,需要药师加强审核,促进临床合理用药. |

| Abstract: | Objective To analyze irrational medication orders of pharmacy intravenous admixture service to improve clinical rational drug use. Methods The irrational medication orders of pharmacy intravenous admixture service from January 2012 to June 2012 were analyzed retrospectively. Results A total of 423 irrational medication orders were found, accounting for 0.87%, including irrational selection of solvent in 47 cases, improper medication frequency in 132 cases, medicine incompatibility in 95 cases, irrational usage and dosage in 79 cases, irrational combined medication in 38 cases and orders-entry error in ：32 cases. Conclusion There are problems in clinical drug use; pharmacists should check the medication orders carefully to improve clinical rational drug use. |
