米非司酮保守治疗异位妊娠的临床及病理研究

目的 :探索米非司酮作为杀胚药保守治疗早期异位妊娠的可行性、疗效及剂量。方法 :根据用药量随机分为 A组口服米非司酮总量 1 50 mg,1 2 h一次 ,每次 2 5mg,首次加倍 ,共 5次服完。B组口服米非司酮总量 30 0 mg,1 2 h一次 ,每次 50 mg,连用 4次后改为 2 5mg,每 1 2 h一次连用4次 ,共 8次服完。结果 :监测血、尿 HCG转阴 ,B超探测盆腔包块消失或明显缩小 ,腹腔内游离液体吸收 ,腹痛及阴道流血消失为治愈。A组 77例 ,保守治疗成功 72例 ( 93.50 % ) ,B组 2 1例 ,保守治疗成功 2 0例 ( 95.2 3% )。结论 :米非司酮可以做为异位妊娠保守治疗的杀胚药 ,其疗效可靠 ,方法简便 ,两组不同用药剂量研究结果显示其治愈率及治愈时间无显著性差异 ( P>0 .0 5)

Clinical and pathological study of treating ectopic pregnancy conservatively by mifepristone

Objective:To evaluate the feasibility,effect and dosage of treating early ectopic pregnancy conservatively by mifepristone used as a killing embryo drug.Methods:According to the dosage,patients were divided into group A and group B.Group A was given totally oral dosage of mifepristone 150 mg with 25 mg once per 12 h and the dose of the first time was double.Group B was given totally oral dosage 300 mg with 50 mg once per 12 h for four times and 25 mg once per 12 h for the other four times.Results:The cure was certain with the HCG of blood and urine turning to negative,pelvis tumor disappearing or shrinking significantly by B ultrasongraphy,peritoneal free fluid being absorbed,abdominal pain and daginal bleeding disappearing.Seventy two out of 77 cases in group A were cured and the conservative cure rate was 93.5%;Twenty out of 21 cases in group B were cured and the conservative cure rate was 95.23%.Conclusion:Mifepristone can be recognized as a killing embryo drug to treat ectopic pregnancy conservatiuely and it is effective and convenient.No significant difference in cure rate and time was found in the two different dosage groups (P>005). 〔
