低频重复经颅磁刺激对精神分裂症顽固性幻听的随机双盲对照研究

目的:观察低频重复经颅磁刺激(rTMS)对精神分裂症顽固性幻听的临床疗效。方法:将80例住院慢性精神分裂症患者随机分为研究组和对照组各40例,2组患者均采用以原抗精神病药物治疗不变,研究组患者给予经左侧颞顶叶低频rTMS刺激;对照组患者给予伪刺激。于治疗前及治疗4周末,应用阳性与阴性症状量表(PANSS)评定疗效。结果:两组患者各有39例完成治疗。治疗后两组患者的PANSS各因子中幻听及一般精神病理症状两项明显下降(P<0.05,P<0.01)。阳性症状两组患者均无明显改善。阴性症状研究组患者下降明显(P<0.05),而对照组患者改善不明显(P>0.05)。治疗后幻听、阴性症状和一般精神病症状研究组患者低于对照组(P<0.05)。结论:低频rTMS能有效改善精神分裂症顽固性幻听症状。

Double- blind randomized controlled trial of repetitive transcranial magnetic stimulation in treatment of auditory hallucination for schizophrenic patients

Objective：To observe clinical efficacy of repetitive transcranial magnetic stimulation（rTMS） in treatment of auditory hallucination for schizophrenic patients. Methods：80 patients with chronic schizophrenia were randomly assigned to experimental group（n = 40） and control group（n = 40）. Besides the original antipsychotic drugs,the patients in experimental group received real low- frequency rTMS in left temporoparietal lobe,while the patients in control group received false rTMS. The efficacy was evaluated with the positive and negative syndrome scale（PANSS）before and 4 weeks after the treatment. Results： 39 patients of each group completed the trial. After the treatment,the scores of auditory hallucination and the general mental pathological symptoms in the two groups were significantly lower than those before treatment（P〈0. 05,P〈0. 01）. The positive symptoms of the two groups had no significant improvement. The negative symptom score of experimental group significantly decreased（P〈0. 05）,while that of control group had no significant improvement（P〉0. 05）. After the treatment,the scores of the auditory hallucination,negative symptom,and the general mental pathological symptoms in experimental group were lower than those in control group（ P〈0. 05）. Conclusions： Low frequency repetitive transcranial magnetic stimulation can relieve the auditory hallucination for the patients with schizophrenia.
