两种术式治疗急性闭角型青光眼中远期疗效比较

目的比较反折式小梁切除术和常规小梁切除术治疗急性闭角型青光眼的中远期疗效和并发症。方法将1999年4月～2006年8月分别实施这两种术式的年龄、术前眼压、青光眼类型及眼局部情况基本匹配的两组患者比较,其中治疗组34例（40眼）采用反折式小梁切除术,对照组36例（40眼）,采用常规小梁切除术。结果反折式小梁切除术组平均随访34个月,完全成功率达90.10%,总成功率达97.25%,常规小梁切除术组平均随访33月,完全成功率达80.24%,总成功率达87.25%,两组比较差异有显著性（P〈0.01）,早期治疗组浅前房及低眼压发生率较对照组高。结论反折式小梁切除术治疗急性闭角型青光眼成本低,操作简单易行,中远期术后并发症少,降眼压效果明显持久,成功率高,是一种值得推广的术式。

The mid-to-longterm curative effect comparison of curing primary glaucoma between the skills of reflected trabecula resection and standard trabecula resection

[Objective] To compare the mid-to-longterm curative effect and complication of curing Primary Glaucoma with the skills of the reflected trabecula resection and standard trabeeula resection [Methods] The intraoperative age, preoperative ocular tension, types of Glaucoma and partial situations of eyes from two groups of patient with the basic matching were compared after operations by two different skills in the course of April, 1999 to August, 2006. In the curing group 34 patients （40 eyes） had the skill of reflected trabeeula resection and 36 patients （40 eyes） had the standard trabecula resection in the control group. [Result] The patients from the group of reflected trabecula resection was 90.10% of wholly successful pereentage and the 97.25% of total success percentage of based on follow-up results. Meanwhile patients from the group of standard trabecula resection was 80.24% of wholly successful percentage and the total suceessful percentage is 87.25%. It was significant difference （P 〈0.01） that the occuring percentage of superficial anterior and low eye pressure in the earlier curing group was higher than that in the control group. [Conclusion] To cure the Primary Glaucoma with the skill of reflected trabecula resection is operated easily and simply with a low cost. Futhermore it has a little complication and an enduring clear effect of lo .wing ocular tension with a higher successful percentage. So the reflected trabecula resection is worth of extending.
