经口气管插管与经鼻气管插管在ICU患者中的应用比较

目的 :比较经口气管插管和经鼻气管插管在ICU患者治疗中应用范围及利弊。方法 :30例经口气管插管 ,设为A组 ,30例经鼻气管插管 ,设为B组。记录插管操作时间、留管时间、主要并发症及优缺点。结果 :A组平均操作时间 4 0 2 7s,B组为 1 0 8 1s ,P <0 0 1 ;A组平均留管时间 2 35d ,B组为 1 4 76d ,P <0 0 1 ;A组并发症为 1 6 7% ,B组为 36 6 7% ,P <0 0 1。结论 :经口插管A组较经鼻插管B组操作简单、快 ,创伤小 ,排痰容易 ,并发症少 ,在ICU患者抢救中是最为可靠和有效的急救措施。但经鼻插管B组病人易耐受 ,留置时间长 ,易固定 ,便于口腔护理 ,清醒病人不影响进食 ,对ICU慢性呼吸疾病治疗具有不可替代的作用

Comparison of endotracheal intubation via the nose and transmouth for patients in ICU

Objective To compare the clinical application of endotracheal intubation via the nose and transmouth for patients in ICU.Method 60 cases were divided into the 30 cases of intubation through the mouth (Group A) and the 30 cases of intubation via the nose (Group B) to Compare the time of insertion,retaining time of the tube,complications and the advantages and disadvantages of the two ways of intubation.Results The average time of insertion of Group A is 40 27 seconds,while it is 108 1 seconds in Group B ( P <0 01).The retaining time of tube in Group A is 2 35 days,in Group B is 14 76 days ( P <0 01).Complications in Group A are 16 7%,in Group B 36 67%( P <0 01).Conclusion Insertion through the mouth is easier,faster,with little injury,easier sputum drainage and less complications than insertion via the nose.It is more dependable and efficient in emergency first aid.Insertion via nose is more endurable,with longer retaining time,easier to stay,with easier mouth nursing, and does not affect food taking for conscious patients.It is more effective in treating chronic respiratory diseases for patients in ICU.
