硬腭截短-咽侧成型术治疗悬雍垂腭咽成型术后复发8例

摘    要:目的对于阻塞性睡眠呼吸暂停低通气综合征(OSAHS)已行悬雍垂腭咽成型术(UPPP)并症状复发者,探讨应用硬腭截短-咽侧成型术进行治疗的可行性。方法8例行硬腭截短-咽侧成型术者手术前、后进行Muller试验、头影测量和多导睡眠监测(PSG)。结果患者症状均有不同程度的减轻,PSG检测7名患者呼吸暂停低通气指数(AHI)和最低血氧饱和度(LSaO2)较术前有改善。结论硬腭截短-咽侧成型术可以有效扩大腭咽间隙,可以作为部分UPPP术后复发患者的补救手术。

Abstract:Objective To investigate the feasibility and clinical value of hard palate shortening in combination with lateral pharyngoplasty(HPS-LPP) in treatment of OSAHS with recurrence after uvulopalatopharyngoplasty(UPPP).Methods HPS-LPP was performed on 8 patients.The pre-and post-operative scores of polysomnography(PSG),Muller test and cephalometric data were compared.Results No apparent complications occurred,and the clinical symptoms were alleviated.Both the apnea hyponea index(AHI) and the lowest SaO2(LSaO2) were improved in 7 cases.Conclusion The volopharyngeal cavity can be effectively enlarged by HPS-LPP,which may be a revision surgery for recrudescent cases after UPPP.
