硬腭截短-咽侧成型术治疗悬雍垂腭咽成型术后复发8例

目的对于阻塞性睡眠呼吸暂停低通气综合征（OSAHS）已行悬雍垂腭咽成型术（UPPP）并症状复发者,探讨应用硬腭截短-咽侧成型术进行治疗的可行性。方法8例行硬腭截短-咽侧成型术者手术前、后进行Muller试验、头影测量和多导睡眠监测（PSG）。结果患者症状均有不同程度的减轻,PSG检测7名患者呼吸暂停低通气指数（AHI）和最低血氧饱和度（LSaO2）较术前有改善。结论硬腭截短-咽侧成型术可以有效扩大腭咽间隙,可以作为部分UPPP术后复发患者的补救手术。

Hard palate shortening in combination with lateral pharyngoplasty in treatment of recrudescent OSAHS patients following uvulopalatopharyngoplasty

Objective To investigate the feasibility and clinical value of hard palate shortening in combination with lateral pharyngoplasty(HPS-LPP) in treatment of OSAHS with recurrence after uvulopalatopharyngoplasty(UPPP).Methods HPS-LPP was performed on 8 patients.The pre-and post-operative scores of polysomnography(PSG),Muller test and cephalometric data were compared.Results No apparent complications occurred,and the clinical symptoms were alleviated.Both the apnea hyponea index(AHI) and the lowest SaO2(LSaO2) were improved in 7 cases.Conclusion The volopharyngeal cavity can be effectively enlarged by HPS-LPP,which may be a revision surgery for recrudescent cases after UPPP.
