用斑点免疫渗滤试验诊断单纯疱疹病毒性脑炎

采用斑点免疫渗滤试验（DIFA）对苏州地区170例散发性脑炎患者240份脑脊液（CSF）标本中单纯疤疹病毒抗原（HSV－Ag）进行检测，结果表明170例散发性脑炎中，经该项检测而能明确单纯疤疹病毒性脑炎（HSE）诊断者42例（24．7％）；240份CSF标本中，HSV－Ag阳性率为35．4％，而其中42例HSE患者的96份CSF阳性率为88．5％，两者差异有非常显著意义（P＜0．001）。将DIFA法检测结果与单纯疤疹病毒抗体检测结果进行比较显示，应用DIFA检测HSV－Ag能在较早期对HSE作出诊断。

Use of Dot Immunofiltration Assay (DIFA) for the Diagnosis of Patients with Herpes Simplex Encephalitis

Objective We have developed a rapid, and sensitive test for early diagnosis of herpes simplex encephalitis (HSE) and to show the pattern of HSE in the so-called sporadic encephalitis (SE) in the area of Suzhou from 1987 to 1995. Methods Herpes simplex virus antigen (HSV-Ag) 'was detected by dot immunofiltration assay (DIFA) in 240 cerebrospinal fluid (CSF) from patients with SE and of 136 CSF from 100 patients with other neural diseases and 20 normal people (as controls). Results 42 patients with HSE (accounting for 24. 7 % of SE) and 3 patients in controls (2. 5 % ) gave HSV-Ag positive result by this assay (P<0. 001). In CSF from patients with SE the HSV-Ag positive rate was 35. 4% (85/240) but from patients with HSE it was 88. 5% (85/96) (PR0. 001). The earliest HSV-Ag positive samples were shown in the 3rd day of disease onset (4 cases). Conclusion Our study indicated that detection of HSV-Ag in CSF by DIFA could be used in the early diagnosis of HSE and that this method has several other advantages.
