腹腔镜联合内镜治疗肝外胆管结石的临床观察

目的：评价腹腔镜联合内镜治疗肝外胆管结石的疗效。方法：回顾性分析2009-03～2012-04收治的67例肝外胆管结石患者的临床资料（观察组），行腹腔镜联合内镜手术治疗，同期行传统开腹手术的患者52例（对照组），比较两组手术时间、术中出血量、胃肠功能恢复时间、住院时间及并发症。结果：两组手术时间及术中出血量比较无统计学差异（P〉0.05），而观察组患者胃肠功能恢复时间及住院时间显著短于对照组，差异具有统计学意义（P〈0.05）。观察组并发症发生率为6.9％，对照组为22.2％，两组比较具有统计学差异（P〈0.05）。未发生其他严重并发症。结论：腹腔镜联合内镜治疗肝外胆管结石的疗效确切，具有创伤小、术后恢复快、并发症少等优点，值得临床推广应用。

Efficacy of Laparoscopy Combined with the Endoscopic in Treatment of Gallbladder and Common Bile Duct Stones

Objective：To evaluate the efficacy of laparoscopy combined with the endoscopic in treatment of gallbladder and common bile duct stones. Methods：Clinical data of 67 patients with gallbladder and common bile duct stones receiving laparoscopy combined with the endoscopic were analyzed retrospectively （observation group）. 52 patients with gallbladder and common bile duct stones were received open surgery （control group）. Operative time, blood loss, gastrointestinal function recovery time, hospital stay and complications were compared. Results：Both operative time and intraoperative blood loss was no significant difference （P 〉 0. 05 ）. Gastrointestinal function recovery time and hospital stay in observation group were shorter than that in control group, the difference was statistical significant （ P 〈 0. 05 ）. The incidence of complication in observation group was 6. 9% ,while 22. 2% in control group, the difference was statistical significant （ P 〈 0.05 ）. There was no other serious complications. Conclusions： Laparoscopy combined with the endoscopic is effective in treatment of gallbladder and common bile duct stones with trauma, faster recovery,fewer complications, it＇s worthy of clinical application.
