超声在肝脾外伤诊断中的价值

目的探讨超声在肝脾外伤诊断中的应用价值。方法超声检查疑为肝脾外伤患者127例,将其诊断与术中对照及对保守治疗患者随访的临床资料进行回顾性分析。结果本组127例患者,超声诊断肝外伤22例,脾外伤101例,肝脾联合伤3例,其中6例肝脾声像图表现未见异常。经手术治疗93例,经保守治疗34例,误、漏诊患者4例,与手术、CT检查对照总诊断符合率约96％（123／127）。结论超声检查快速,简便,准确率高,是急症肝脾外伤的首选检查方法,在保守治疗随访中也起到积极有效作用。

Value of ultrasonography in the diagnosis of liver and splenic trauma

Objective To evaluate the diagnostic value of ultrasonography in liver and splenic trauma. Methods The ultrasonography of 127 cases sepected with liver and spleic trauma clinically was analyzed retrospectively and with the results of operation and follow-up in non-operation patients. Results In 127 cases, ultrasonography diagnosed 22 cases with liver trauma, 101 cases with splenic trauma ,3 cases with liver and splenic trauma, and 6 cases with nonud appearance. Ninety-three cases were operated and 34 cases were cured conservatively.Among them 4 cases were misdiagnosed. Compared with the results of operation and CT, the diagnostic accuracy of ultrasonography was 96%（123/127）. Conclusion As a fast and easy method with high accuracy, ultrasonography can be considered as the first choice for diagnosing liver and splenic trauma and following up for non-operation patients.
