颈椎过伸性损伤的手术治疗

目的：探讨颈椎过伸性损伤手术治疗方法的选择及疗效.方法：72例颈椎过伸性损伤患者中,11例行颈后路全椎板切除减压术,40例行颈前路减压、植骨＋钢板内固定术,21例一期颈前路减压、植骨＋钢板内固定,二期后路椎板减压术.结果：平均随访26.4个月,8个月内植骨均获得骨性融合,无钢板、螺钉松动、断裂等并发症.术前、术后1个月及最后随访时脊髓运动功能ASIA评分分别为32.54、56.47、85.28分;感觉功能评分分别为71.20、110.73、191.46分,改善有统计学意义.结论：合理选择手术方案,可促进颈椎过伸性损伤神经功能的恢复.

Surgical treatment of cervical hyperextension injury

Objective:To evaluate the methods and efficacy of surgical treatment of cervical hyperextension injury.Method:Seventy-two cases of cervical hyperextension injury were retrospectively analyzed.11 cases were treated with posterior laminectomy,40 cases were treated with anterior disectomy or corpectomy,bone graft and plate fixation.21 cases were treated with anterior decompression and plate instrumentation on the first stage and then posterior laminectomy was done three to six months later.Result:The average follow-up time was 26.4 months,all bone grafts acquired fusion in eight months,no plates or screws broken or loosen.The ASIA motor function and sensory function improved from 32.54,71.20 at pre-operation to 85.28,191.46 at the last follow-up,respectively.Conclusion:The motor and sensory function will be improved significantly if the treatment of cervical hyperextension injury can be selected properly.
