在社区开展高血压“二级预防”措施的探讨

目的：评价以社区义务血压测量员持续监测血压为主要干预手段的社区高血压“二级预防”管理的效果，探索高血压患者社区管理的有效途径。方法：按知情同意原则选择被医疗机构确诊的高血压患者，在社区居委会接受血压监测；在社区居委会培训义务血压监测员，由测量员每2周对患者血压实施监测上报，社区医生针对性地开展一对一用药和行为指导。结果：患者血压控制率由14．7％上升为67．4％，干预后血压水平有明显下降。结论：在社区居委会开展血压监测工作是有效完善“三级预防”网络的一项可行措施，可以减轻医务人员负担，方便社区高血压患者，营造社区支持环境，提高监测管理效能。

Objective： Evaluate the effect of the obligation blood pressure surveyor inspect the hypertension inside the community and try to explore how to manage the hypertension sufferer inside the community. Methods： With the agreement of hypertension sufferer who had been diagnosed by the hospital, community will inspect the blood pressure change. The surveyor will report the results every two weeks, the community doctor will treat the suffer use different way according to their different status. Results：The under control percentage of the suffer increased from 14.7 % to 67.4%, the same time sufferer blood pressure drop a lot. Conclusion： To develop the inspection work inside the community can decrease the hospital pressure, convenient the sufferer and consummate ＂three levels prevent＂ network.
