妊娠合并系统性红斑狼疮的妊娠结局及其影响

目的：探讨妊娠合并系统性红斑狼疮(SLE)的妊娠结局及其影响因素。方法：对1995-01～2002-12收治的10例妊娠合并SLE的临床资料进行回顾性分析。结果：10例中，活动型2例，稳定型8例。妊高征合并胎儿宫内窘迫2例；早产5例；胎儿生长受限2例；胎膜早破3例，其中1例合并宫内感染；新生儿轻、重度窒息各1例。新生儿均存活。结论：妊娠合并SLE的妊娠结局与SLE临床分型、妊娠前SLE是否缓解、妊娠期SLE病情控制程度以及强的松用量等因素有关。多学科共同监护可以改善其妊娠结局。

Outcome of Pregnancy with Systemic Lupus Erythematosus and Its Influence Factors

Objective;To study the outcome of pregnancy with systemic lupus erythematosus and its influence factors. Methods: Retrospective analysis of the clinical data of 10 pregnancies with systemic lupus erythematosus between Januay 1995 and December 2002 was performed. Results: Among ten patients, two were active SLE and the other eight were quiescent SLE. Pregnancy outcomes and obstetric complications included : pregnancy-induced hypertension syndrome accompanied with fetal distress,2 cases; preterm delivery,five cases; intrauterine growth restriction,2 patients; premature rupture of membranes,three patients, one of them was accompanied with utero infection; one neonatal mild asphyxia and one neonatal serious asphyxia. All neonatal survived. Conclusion:Outcomes of pregnancy with systemic lupus erythematosus were associated with the following :clinical types of SLE, whether patients were in clinical remission or not before pregnancy, the degree of controlling disease activity during pregnancy and the dose of prednisone. Pregnancy outcome could be improved by careful multidisciplinary monitoring.
