小肠移植后淋巴管重建对大鼠生存和脂肪酸吸收的影响

摘    要:小肠淋巴管系统的功能涉及脂肪和一些药物的吸收、小肠淋巴液的调节、大分子的交换和淋巴细胞的迁移。小肠移植后可致移植引流淋巴管的完全离断,但其带来的影响尚不清楚。本研究在大鼠小肠移植显微外科淋巴管重建(LR)模型研究其对移植大鼠生存率、血浆脂肪酸和血浆FK506水平的影响。异位双造口小肠移植在BN和LEW或LEW和LEW大鼠间进行。分为6组:AI组,BN+LEW+LR;A2组,BN+LEW-LR;B1组,BN+LEW+LR+PK506;B2组,BN+LEW-LR+FK506;C1组,LEW+LEW-LR;C2组,LEW+LEW+LR。淋巴管重建在移植物胸导管宿主乳糜池间,由淋巴管造影评定通畅率。结果显示:①排斥反应不影响重建淋巴管的通畅率;②小肠移植后淋巴管重建显延长用FK506控制排斥反应宿主的平均生存时间和增加生存大鼠的体重;③小肠移植后的淋巴管重建显增加术后早期脂肪酸的吸收,在术后14天后与未行淋巴管重建宿主的血浆脂肪酸水平无显性差异;④小肠移植后淋巴管重建,使宿主较未行淋巴管重建大鼠术后血FK506水平高而迅速。因此,我们的初步研究表明,小肠移植后淋巴管重建通过增加脂肪酸吸收和FK506的吸收对宿主大鼠产生有益的影响。而LR在小肠移植中的确切作用有待进一步研究。

The effects of lymphaticreconstruction on survival and fatty acid absorption after small bowel transplantation inrat
Abstract:The functions of the lymphatic system involved in lipid andremedy absorption the regulation fo interstitial fluid,macromolecule exchange,macromolecule exchange and migration of lymphocytes.The surgical procedure of small bowel transplantation results in the complete disruption of the graft lymphatic drainage,and the consequences are still uncertain.This investigation aims to understanding the effects of lymphatic reconstruction (LR) on survival and fatty acid absorption after small bowel transplantation rat.BN or LEW small bowel graft transplant into LEW rats either with or without LR.Animals were divided into six groups:group A1,BN+LEW+LR;A2,BN+LEW-LR;B1,BN+LEW+LR+FK506;B2,BN+LEW-LR+FK506;C1,LEW+LEW+LR;C2,LEW+LEW-LR.In the LR model,the thoracic duct of the donor was sutured to the recipient's cisterna chyli.The patency of the LR was assessed by methylene-blue staining.The results:①Rejection did not affect the patency rate of LR;②LR significant increased mean survival time and body weight gain of recipients who treated with FK506;③LR significant increased recipient's fatty acid absorption during early period of postoperation.but no significant different with recipients who without LR after 14 day postopertaion;④LR increased recipient's plasma level of FK506 significantly and quickly.We conclude that LR improved recipient's fatty acid and FK506 absorption and body weight gain,improved the long-term serult after small bowel transplantation in rats.The precise role of LR after small bowel transplantation needs to be further investigated.
