超声与CT在脾动脉瘤诊断中的应用价值

目的探讨超声与CT成像对脾动脉瘤的诊断价值。方法回顾性分析经手术、病理证实的20例脾动脉瘤患者的超声及CT表现。结果超声漏诊1例,误诊1例,准确诊断15例,15例位于脾门处的脾动脉瘤彩色多普勒超声表现为瘤体内红蓝色血流信号各半,界限分明的半涡流信号,与脾动脉相通。位于脾动脉起始端和中段的3例超声只发现病变,未具体诊断为脾动脉瘤,超声诊断符合率75%。术前20例CT全部诊断为脾动脉瘤,与手术和病理完全一致,CT诊断符合率100%。结论超声对脾动脉瘤的诊断准确率低于CT,但彩色多普勒超声对于脾门处脾动脉瘤诊断具有较高特异性,且方便、快捷,是初诊筛选本病的首选检查方法;CT检查对该病变诊断具有很高的准确率,是术前确诊该病变的必须的检查方法。

Application value of ultrasonography and CT in diagnosis of splenic artery aneurysms

Objective To study the value of ultrasonography and CT in diagnosing splenic artery aneurysms. Methods Twenty patients with surgically and pathologically proved splenic artery aneurysm underwent preoperative ultrasonography and CT examination. Results One case was miss diagnosed and 1 case was missed diagnosed by ultrasound, 15 cases were accurately diagnosed by ultrasound. The ultrasound of the 15 cases showed half red and half blue eddy current with clear boundary in the splenic hilum, which was communicated with the splenic artery. 3 cases with lesions in the beginning and middle segment of the splenic artery were discoverd but not accurately diagnosed by ultrasound. The accuracy of ultrasoud was 75%. All 20 cases were definitely diagnosed by CT preoperatively. CT findings of all patients were corresponding to those of surgery and pathology, the diagnostic accuracy of CT was 100%. Conclusion Ultrasonography in the diagnosis of splenic artery aneurysms has higher specificity and the procedure is convenient and quick, so that it should be the first choice to diagnose this disease. And CT is the necessary choice preoperatively.
