纤维支气管镜在诊治外伤后肺不张中的应用价值

目的探讨纤维支气管镜（纤支镜）对外伤后肺不张的诊治价值。方法对12例外伤后肺不张患者，在吸氧及心电监护和经皮血氧饱和度监测下行纤支镜检查；表面麻醉后，纤支镜插至支气管阻塞部位抽吸分泌物、痰栓及血凝块，较大血凝块用活检钳夹碎吸出；用生理盐水分次灌洗，稀释粘稠分泌物，反复吸引，直至清除；并在气道内注入适当的抗菌素。结果纤支镜治疗后，不张肺全部复张，无一例发生严重并发症。结论纤支镜诊治外伤后肺不张安全有效，并发症少。

Value of Fiberbronchoscopy in Diagnosis and Treatment of Post-traumatic Atelectasis

Objective To explore the clinical value of fiberbronchoscopy in the diagnosis and treatment for post-traumatic atelectasis. Methods Fiberbronchoscopy examination was performed in 12 cases with post - traumatic atelectasis under the monitoring of inspiration oxygen, ECG and transcutaneous blood oxygen saturation. After superficial anesthesia, the fiberbronchoscopy were inserted to bronchial obstructive place. Secretion and hard clot were aspirated, mucus was diluted with normal saline, and an- tibiotic were infused. Results All atelectasis recovered after fiberbronchoscopy operation. No severe complications occurred. Conclusions Fiberbronchoscopy operation is safe and effective for post - traumatic atelectasis.
