通过端侧缝合克服周围神经缺损的实验研究

摘    要:
目的 研究将一有缺损神经的近端和远端与同一正常神经干行端侧缝合后,近端神经纤维沿该神经干克服缺损进行再生的可能性。方法 A组,将兔的左侧尺神经切除3cm,远近端均与正中神经干行端侧缝合,右侧以正常尺神经作为对照,B组,左侧神经处理同A组,右侧尺神经行同样长切除后进行神经移植作为对照,术后6个月通过神经电生理和组织学检查了解神经再生的可能性及再生质量。结果 在两组左侧,电刺激尺神经近段以及近侧端侧缝

Abstract:
Objective To study the possibility of nerve regeneration along normal nerve trunk to overcome nerve defect, when both ends of a defected nerve is sutured end to side with the common normal nerve Methods Group A, a three centimeter long segment wsa removed from the rabbit ulnar nerve on the left side, both ends of it are sutured end to side with the median nerve trunk On the right side the ulnar nerve was undisturbed as normal control; Group B, on the left side the nerves were treated as in group A, on the right side a three centimeter long ulnar nerve defection was also made and then grafted as control Six months later electrophysiological and histological examination were made to know the possibility and quality of regeneration Results one the left side in both group, stimulation of the proximal ulnar nerve, and the median nerve proximal to the proximal nerve suture site, may induce M waves in the flexor carpal ulnaris muscle In group B, electrophysiological examination and nerve image analysis show the result of nerve regeneration by the two terminolateral neorrhaphy way of repair is no better than that by the nerve graft Conclusion When both ends of a defected nerve is sutured end to side with the common normal nerve trunk, nerve regeneration may occur across the defect along the normal nerve trunk; The regenerating fibers in the distal injured nerve have two sources, reimmervation of the proximal injured nerve and collateral sprouting from the normal nerve trunk
