辨证分期联合西药治疗糖尿病足32例临床观察

[目的]观察辨证分期联合西药治疗糖尿病足疗效。[方法]采用前瞻性设计方法,对32例住院患者①控制饮食、控制感染、抗凝、改善循环与微循环、恢复神经功能等常规治疗。②中医辨证分期:湿热壅盛期四妙勇安汤和萆薢渗湿汤(当归、金银花、连翘、玄参、蒲公英、牛膝、赤芍、川芎、萆薢、制乳香、制没药、甘草),1剂/d,水煎300mL,3次/d;热毒缓解期顾步汤(黄芪、当归、党参、牛膝、金银花、玄参、石斛、蒲公英、鸡血藤、甘草),1剂/d,水煎300mL,3次/d;恢复期桃红四物汤合十全大补汤(熟地、当归、白芍、川芎、桃仁、红花、牛膝、党参、白术、茯苓、菟丝子、甘草),1剂/d,水煎300mL,3次/d。连续治疗14d为1疗程。观测临床症状、创面愈合,肢体末梢血液循环、不良反应。治疗1疗程,判定疗效。[结果]临床治愈13例,显效8例,有效8例,无效3例,总有效率90.6%。[结论]辨证分期联合西药治疗糖尿病足效果显著,值得推广。

Clinical Observation on 32 Cases of foot by Traditional Chinese Medicine Combined with Western Medicine in the Treatment of Diabetes

Objective：To observe the treatment according to syndrome differentiation combined with western medicine in the treatment of diabetic foot. [method] using a prospective design method,32 patients hospitalized in the control diet,the conventional treatment control infection,anticoagulation,improve circulation and microcirculation,the recovery of nerve function. The TCM syndrome differentiation and staging：heat dampness period four wonderful Yongan Decoction and Bixie Shenshi soup（Chinese angelica root,honeysuckle,forsythia,Radix Scrophulariae,dandelion,Rhizoma Chuanxiong,radix paeoniae rubra,radix,Bixie,frankincense,myrrh,licorice）,1 doses of /d,water simmer in water 300 mL,3 times /d;heat toxin remission Gu step Decoction（Huang Qi,angelica,Codonopsis,radix,honeysuckle,figwort,Dendrobium,dandelion,Caulis Spatholobi,licorice）,1 doses of /d,water simmer in water 300 mL,3 times /d;recovery Taohong Siwu Decoction Combined Shiquan Dabu Decoction（radix rehmanniae,Radix Paeoniae Alba,Rhizoma Chuanxiong,angelica,peach kernel,safflower,Radix Codonopsis,Atractylodes,Poria cocos,dodder seed,licorice）,1 doses of /d,water simmer in water 3 times 300 mL,/d. Continuous treatment with 14 d for 1 courses. Observation of clinical symptoms,wound healing,limb peripheral blood circulation,adverse reaction. 1 courses of treatment,the curative effect. [Results] the clinical cure 13 cases,markedly effective in 8 cases,effective 8 cases,invalid 3 cases,the total efficiency of 90.6%. [Conclusion] by traditional Chinese medicine combined with western medicine in the treatment of diabetic foot effect is significant,worthy of promotion.
