常熟市胃癌危险因素的病例对照研究

摘    要:
[目的]探索常熟市胃癌的高危因素,为采取有效的预防措施提供科学依据。[方法]选择常熟市2005年8月至2006年8月期间诊断的男性新发胃癌病例202例,按年龄、性别和居住地进行以人群为基础的1∶1配对病例对照研究,获得环境暴露等资料。分别采用Epi-info(6.04版)和SAS(6.12版)统计软件进行单因素和多因素条件Logistic回归分析,计算各因素的调整比值比(OR)和95%可信限(95%CI)。[结果]单因素分析发现,多吃腌菜(近10年和10年前)、多吃咸鱼咸肉(近10年和10年前)、近10年口味咸、三餐不按时吃、饮茶(浓或较浓)、患病前精神压抑、性格内向、胃溃疡或萎缩性胃炎史等因素与胃癌成正相关(P<0.05),而多吃新鲜蔬菜(近10年和10年前)和生活态度乐观能降低患胃癌的危险(P<0.05)。多因素条件Logistic回归分析结果显示,胃溃疡和萎缩性胃炎史、三餐不按时和10年前多吃腌菜是胃癌的主要危险因素,而10年前多吃新鲜蔬菜则作为胃癌保护因素进入回归方程。[结论]针对上述胃癌危险因素进行预防已显得十分必要。

Abstract:
[ Objective] To study the risk and protective factors for stomach cancer in Changshu. [ Methods] Population based case - control study was conducted in Changshu. 202 new cases of stomach cancer from August 2005 to August 2006 were matched in 1:1 with normal population controls. Epi - info(6.04) and SAS (6.12) were used in data analysis. [ Results ] The results of single factor analysis indicated that more salted vegetables intake ( in recent ten or more years ), more salted fish and meat intake ( in recent ten or more years ), more salty flavoring, irregular time for meals, drank thick or thicker tea, with constrained spirit, introverted disposition, history of gastric ulcer and (or) atrophic gastritis were risk factors of stomach cancer. In contrast, more fresh vegetables intake (in recent ten or more years} and optimistic character could decrease the risk for stomach cancer. Result from multiple conditional logistic regression analysis showed that history of gastric ulcer and ( or ) atrophic gastritis, irregular time for meals , more salted vegetables intake in recent ten years were risk factors for stomach cancer, while more fresh vegeta- bles intake in recent ten years was a protective factor for stomach cancer. [ Conclusion] It is necessary to prevent the above risk factors of stomach cancer.
