多导管负压吸引加人工气腹治疗多发肺大疱的临床研究

摘    要:
目的 探讨多导管负压吸引加人工气腹治疗多发肺大疱的手术方式和疗效。方法对36例多发肺大疱患者进行治疗前后肺功能、6min行走距离(6MWD)测定,并用简易医学研究委员会呼吸困难评分标准和美国医学结局研究组研制的简明健康调查问卷(SF-36)标准对呼吸困难程度及生活质量进行评价。结果与治疗前比较,治疗后用力肺活量(FVC)、1秒钟用力呼气容积(FEV1)、最大通气量(MVV)及6MWD均较治疗前增加(P〈0.01);呼吸困难分级下降(P〈0.01);SF-36问卷评分增高(P〈0.05或P〈0.01);肺大疱大部闭合或完全闭合,治疗总有效率这94.4%。结论多导管负压吸引加人工气腹治疗多发肺大疱,可有效改善患者心肺功能和生活质量,且操作简便、经济实用,为不适合剖胸手术治疗的多发肺大疱患者开辟了一条新的途径,值得临床推广应用。

Abstract:
Objective To tiple pulmonary bulla with multiple study the (operatius) program and curative effect of treatment of mulcather and vacuum suction in combination with artificial pneumoperitoneum. Methods Using 36 cases of multiple pulmonary bulla patients, conduct measurement on their pulmonary function ,6 -minute walk distance (6MWD) before and after treatment, and assess their dyspnea grade and living quality with the modified dyspnea scale of English Medical Research Council and short form 36 health survey questionnaire ( SF - 36) of American Medical Outcomes. Results After therapy, FVC, FEV1, MVV and 6MWD were all enhanced (P 〈 0.01 ) ; Dyspnea grade was declined (P 〈 0.01) ;SF -36 score was improved (P 〈 0.05 or P 〈 0. 01) , effective rate of treatment was all 94.4%. Conclusions The therapy of multiple pulmonary bulla with multiple cather and vacuum suction in combination with artificial pneumoperitoneum can improve patients' cardiopulmonary function and living quality effectively.
