微波消融治疗脊柱转移瘤临床指南

脊柱是恶性肿瘤最常见的骨转移部位。近年来, 微波消融技术已逐渐应用于脊柱转移瘤的治疗并取得了良好的治疗效果。但是在微波消融治疗脊柱转移瘤的临床应用方面仍存在一些争议, 包括适应证的选择、消融的功率、消融时间以及温度的控制等。为规范治疗、提高疗效、降低手术相关的并发症发生率, 中国抗癌协会肿瘤微创治疗专业委员会骨与软组织肿瘤学组和中华骨科杂志编辑部组织骨与软组织肿瘤及介入领域的方法学专家制订本临床治疗指南。本指南采用推荐意见分级的评估、制订及评价(Grading of Recommendations Assessment, Development and Evaluation, GRADE)方法和卫生实践指南报告标准(Reporting Items for Practice Guidelines in healthcare, RIGHT)遴选出我国骨科、肿瘤科和介入科医生最为关注的15个临床问题, 最终形成15条循证医学推荐意见, 旨在提高微波消融在脊柱转移瘤治疗的规范化与科学性。

Clinical guideline for the microwave ablation in treating spinal metastases

Spinal metastases are the most common source of morbidity in patients with malignant cancer.In recent years,the microwave ablation technology has emerged and produced satisfactory results in the management of spinal metastases.However,there is still controversy in terms of clinical treatment,such as indication,power,time and temperature.In order to standardize the application of microwave ablation technology and reduce the risk of surgical-related complications for spinal metastases,we summarized the current evidence and clinical experience of microwave ablation and developed a clinical guideline,which was initiated by the Musculoskeletal Tumor Group of Committee of Minimally Invasive Therapy in Oncology,Chinese Anti-Cancer Association and the Editorial Office of Chinese Journal of Orthopaedics.The Grading of Recommendations Assessment,Development and Evaluation(GRADE)approach was used in this specific case to rate the quality of evidence and the strength of recommendations,and the Reporting Items for Practice Guidelines in Healthcare(RIGHT)checklist was strictly followed to report the guideline.Finally,15 evidence-based recommendations were formulated based on 15 most concerned clinical questions among orthopaedic surgeons,oncologists and interventional radiologists in China.This guideline is intended to improve the normalization and scientificity for treatment of spinal metastases using microwave ablation.
