涤痰通脉汤治疗痰瘀互结型卒中后抑郁临床观察

目的:观察涤痰通脉汤治疗痰瘀互结型卒中后抑郁的疗效性及安全性。方法:选取72例符合条件的患者,随机分为两组各36例。两组均予脑卒中常规治疗;对照组另予盐酸文拉法辛治疗,治疗组另予用涤痰通脉汤治疗,两组疗程均为4周。疗程结束后比较两组治疗前后汉密尔顿抑郁量表(HDRS)评分、中医证候积分和临床疗效。结果:治疗组与对照组HDRS评分的总有效率分别为86.11%和80.56%;中医证候总有效率分别为91.67%和77.78%;治疗组临床疗效明显优于对照组(P﹤0.05)。两组治疗前后的HDRS评分、中医证候积分均明显降低(P﹤0.05);且治疗组HDRS评分、中医证候积分明显低于对照组(P﹤0.05)。对照组有5例患者出现口干,治疗组未出现不良反应。结论:涤痰通脉汤治疗痰瘀互结型卒中后抑郁相对盐酸文拉法辛有较好的疗效。

Clinical observation of Ditan Tongmai decoction on post-stroke depression of phlegm and blood stasis syndrome

Objective:To observe the clinical effect and safety of Ditan Tongmai decoction on post-stroke depression of phlegm and blood stasis syndrome.Methods:72 cases of eligible patients were selected and randomly divided into two group,36 cases in each group.Both groups received the conventional treatment for stroke.On the basis of this,the control group was treated plus with venlafaxine hydrochloride,while the treatment group was treated plus with Ditan Tongmai decoction.For both groups,the course of treatment was 4 weeks.After the treatment,Hamilton Depression Rating Scale(HDRS)score,TCM syndrome score and clinical effect were compared between two groups before and after treatment.Results:In treatment group and control group,the total effective rate of HDRS score was 86.11%and 80.56%respectively;the total effective rate of TCM syndrome was 91.67%and 77.78%respectively;the clinical effect of treatment group was significantly better than that of control group(P﹤0.05).After treatment in both groups,HDRS score and TCM syndrome score were significantly reduced compared before treatment(P﹤0.05);and HDRS score and TCM syndrome score of treatment group were significantly lower than those of control group(P﹤0.05).5 cases of patients in control group were with xerostomia;there were no adverse reactions in treatment group;Conclusion:Ditan Tongmai decoction has better curative effect on post-stroke depression of phlegm and blood stasis syndrome than venlafaxine hydrochloride.
