腹腔镜与开腹手术在结直肠癌治疗中的临床分析

目的评价腹腔镜在结直肠癌根治术中的安全性,并与传统开腹手术就手术效果、术后临床恢复等情况进行对比研究。方法分析既往诊治的非转移性结直肠癌患者131例,其中选择腹腔镜下结直肠癌根治术的患者(LCS组)共53例,同期开腹结直肠癌根治术患者(OCS组)78例,详细收集每例患者的数据资料。就两组手术操作的安全性、术后肿瘤学效果以及恢复等进行对比分析。结果两组相比较,在手术时间、术中出血、淋巴结清扫个数、瘤体远切端距离及术后并发症发生率方面均无统计学差异(P>0.05);而在切口长度、术后恢复时间以及术后住院时间上具有显著性差异(P<0.05)。结论运用腹腔镜技术行结直肠癌根治术,具有操作安全、创伤小、恢复快等特点,不仅技术上可行,而且可达到与开腹根治术相近的效果,是临床中可供选择应用的治疗手段。

Clinical analysis of laparoscopic and open surgery in the treatment of colorectal cancer

Objective To evaluate the safety of laparoscopic radical on colorectal cancer patients,and compare the effects and postoperative recovery with traditional open surgery.Methods The clinical data of 131 patients with colorectal cancer received operation in this hospital were retrospectively analyzed.All these patients were divided into two groups,namely laparoscopic colorectal surgery group（LCS group,n=53） and open colorectal surgery group（OCS group,n=78）.The safety of surgical procedures,postoperative oncological effect and recovery were compared between these two groups.Results There was no significant difference between these two groups with regard to operating time,intraoperative blood loss,number of lymph nodes resected and rate of postoperative complications（P0.05）.However there was significant difference between these two groups with regard to length of incision,time for postoperative recovery and duration of hospitalization（P0.05）.Conclusion Laparoscopic techniques for colorectal cancer surgery is safe,minimally invasive and quick recovery.It can achieve the similar radical effect versus open surgery,and it is a choice for colorectal cancer patients in clinic.
