中医“三联”外治法治疗神经根型颈椎病临床疗效观察

目的：分析中医手法推拿、牵引、中药熏蒸等＂三联＂外治法对神经根型颈椎病患者的治疗效果,探讨手法推拿、牵引、中药熏蒸治疗神经根型颈椎病的机理。方法：2004年7月～2010年6月选择160例甘肃中医学院附属医院住院的神经根型颈椎病患者,随机分为治疗组和对照组,每组80例,对治疗组进行手法推拿、牵引、中药熏蒸治疗;对照组进行牵引及液体疗法。采用疼痛综合评定、症状与体征改善评定,分别在治疗前后对2组患者进行评价。结果：160例患者,随访12～18月,治疗组有效率高于对照组（P〈0.05）,治疗组与对照组治疗前后在疼痛积分差值、体征试验阳性率、临床症状、体征积分差值方面比较,差异有统计学意义（P〈0.05）。结论：中医手法推拿、牵引、中药熏蒸等＂三联＂外治法通过恢复脊柱力学平衡,可改善颈部肌肉的力学失衡状态,从而达到缓解神经根型颈椎病的作用。

Clinical Observation of Three United Remedy of TCM External Therapy in Treating Nerve Root Type Cervical Spondylosis

Objective:To observe the therapeutic effect of three united external treatment(Chinese massage techniques,traction,and traditional Chinese medicine fumigation) on nerve root type cervical spondylosis,and to explore the mechanism.Methods: All 160 cases of nerve root type cervical spondylosis were selected in Gansu hospital of TCM from July 2004 to June 2010,and they were randomly divided into treatment group and control group,with 80 cases in each group.The patients of the treatment group were treated with massage,traction,and traditional Chinese medicine fumigation.And the patients of the control group were treated with traction and fluid therapy.The comprehensive assessment of pain and improvement of symptoms and signs were evaluated in the two groups before and after treatment.Results: All patients were followed up for 12~18months.The efficiency in treatment group was higher than the control group(P<0.05).The scores of pain,the positive rate of signs test,the symptoms and signs points differences were significant difference between the two groups before and after treatment(P<0.05).Conclusion:The three united remedy of TCM external therapy(Chinese massage techniques,traction,and traditional Chinese medicine fumigation) is effect to relieve symptoms of nerve root type cervical spondylosis by restoring the spine mechanical balance and improving the neck muscles mechanical imbalance.
