乳腺癌患者术后疾病不确定感与焦虑抑郁及应对方式相关性研究

目的探讨乳腺癌术后住院患者疾病不确定感水平、焦虑抑郁状况、应对方式及三者之间的关系。方法采用Mishel的疾病不确定感量表(Mishel’s Uncertainty in Illness Scale,MUIS)、焦虑自评量表(Self-Rated Anxiety Scale,SAS)、抑郁自评量表(Self-Rated Depression Scale,SDS)及医学应对问卷(Medical Coping Modes Questionnaire,MCMQ)对55名乳腺癌术后住院患者进行问卷调查。结果87.27%的患者疾病不确定感处于中等水平;面对应对方式使用最多,屈服应对方式使用最少;患者的焦虑发生率为84.10%,抑郁发生率为89.50%。疾病不确定感与面对应对方式呈负相关(P<0.01),与回避和屈服两种应对方式呈正相关(P<0.05),与焦虑、抑郁均呈正相关(P<0.01)。结论护士应指导患者采取有效的应对方式降低疾病不确定感,从而减轻患者的心理压力,提高生活质量。

Study on the correlations between postoperative uncertainty in illness and anxiety, depression, coping mode in breast cancer patients

Objective To determine the level of postoperative uncertainty in illness,anxiety,depression,coping mode and their correlations in breast cancer patients.Methods 55 patients were assessed with Mishel's Uncertainty in Illness Scale(MUIS),Self-Rated Anxiety Scale(SAS),Self-Rated Depression Scale(SDS) and Medical Coping Modes Questionnaire(MCMQ).Results 87.27% of the patients showed moderate uncertainty in illness.Most of them adopted confrontation coping mode rather than resignation in illness.84.10% and 89.50% of the patients showed anxiety and depression respectively.The uncertainty in illness was negatively related to confrontation coping mode(P<0.01),positively related to avoidance and resignation(P<0.05),and positively related to anxiety and depression(P<0.01).Conclusion Nurses should instruct patients to adopt effective coping modes to decrease the uncertainty in illness,relieve patients' psychological stress and improve the quality of life.
