微卡菌苗预防糖尿病患者并发肺结核的临床观察

摘    要:
目的探讨母牛分支杆菌菌苗(微卡)对糖尿病患者预防并发肺结核的临床疗效及安全性。方法随机配对分组法将80例糖尿病患者分入微卡治疗组(M组,40例)和对照组(C组,40例)。两组常规使用降糖药及饮食控制血糖。M组加用微卡治疗3个月,C组不用微卡。结果M组未见微卡对血糖控制等产生不良影响,细胞免疫功能显著改善,治疗后M组T细胞亚群各项指标与C组比较:CD3、CD4显著升高(P〈0.001),CD8显著下降(P〈0.05);PPD皮试阳转率M组较C组显著提高(P〈0.05);近2年观察M组无1例发生肺结核,C组有4例发生。结论微卡临床使用安全,有显著改善糖尿病患者的细胞免疫功能对并发肺结核有较好的预防作用。

Abstract:
Objective To investigate the effect and safety of Mycobacterium vaccine ( M Vaccae vaccine) on patients with diabetes from tubereulosis. Methods 80 cases with diabetes from tuberculosis were divided into immunotherapy ( M ,40 cases) group and control (C ,40 cases) group at random pair. Both of them were given diabetes drugs and diet treatment. Group M was treated with immunotherapy with M vaccac vaccine for 3 months while Group C wasn't. Results The side effects of M vaccae vaccine were found in Group M. Compared with group C, group M were better in improving of T cell's standers : increasing of the percentage of CD3 and CD4 Tcell ( P 〈 0. 001 ), and decreasing of CD8 T cell (P 〈 0. 05 ) ; The PPD test negative conversion rates of group M were significantly higher than those of Group C ( P 〈 0.05 ). No case relapsed in Group M, while 4 case were found in Group C during the 2 year followup. Conclusions Immunotherapy with M vaccae vaccine is safe in clinical appliance. It can improve the diabetes patients' T cell function. It might play a role in prevention of patient with diabetes from tuberculosis.
